| Coordinates and facilitates the intake process for home care patients. Provides information about home care services, eligibility criteria, and policies to patients and families. Utilizes health record management and other software to process the flow of documentation required to schedule and deliver services to patients. Responds to questions and resolves issues to ensure customer satisfaction. Requires a high school diploma or equivalent. Typically reports to a manager or head of a unit/department. Works under moderate supervision. Gaining or has attained full proficiency in a specific area of discipline. Typically requires 1-3 years of related experience. |
